General Dog Release Waiver

Please read the following:

 

Champions Premier Dog Parlor cares about your pet’s safety. We want to assure you that every precaution is taken to provide a safe and stress-free grooming experience for your pet.

 

Due to the intimate nature of the grooming process and/or a pet’s tolerance level for stress, strangers, or handling, some pets can become anxious or reactive during the grooming service.

 

If we determine that continuing a grooming service would present a threat to your dog’s emotional or physical health and/or the safety of the groomer completing the service, we will discontinue that day’s service and contact you immediately.

 

Champions Premier Dog Parlor takes all necessary precautions to complete the grooming service safely and humanely, including the proper use of common industry safety equipment, such as grooming loops, muzzles, and restraints.

 

Occasionally, the grooming process can expose a hidden medical problem or aggravate a current one. This can occur during or after grooming. In the best interest of your pet, we request your permission to obtain immediate veterinary treatment should it become necessary.

 

If your pet is severely tangled or matted, it is at greater risk of injury, stress and trauma. All precautions will be taken, however, problems occasionally arise during or after grooming. Such problems include, but are not limited to, nicks, clipper irritation and mental or physical stress. In the best interest of your pet, we request your permission to obtain immediate veterinary treatment should it become necessary.

 

 

Please read the following:

 

I hereby grant permission to Champions Premier Dog Parlor to utilize proper restraints for the health and safety of my pet and my grooming service provider during today’s grooming service. I understand and agree that if Champions Premier Dog Parlor deems it unsafe or unhealthy to continue for the pet or the groomer, Champions Premier Dog Parlor will end the service and I will be responsible for any services rendered.

I hereby grant permission to Champions Premier Dog Parlor to obtain emergency veterinary treatment for my pet at my expense


I hereby understand that matted pets have a greater chance of injury during grooming. I will not hold Champions Premier Dog Parlor responsible for any accident or injury to my pet.

Your appointments are very important to the team members of Champions Premier Dog Parlor. We understand that sometimes schedule adjustments are necessary; therefore, we respectfully request at least 24 hours notice for cancellations by calling directly at 484-887-8800. Please understand that when you forget or cancel your appointment without giving enough notice, we miss the opportunity to fill that appointment time, and clients on our waiting list miss the opportunity to receive services. You will receive an appointment confirmation 48 hours in advance because we know how easy it is to forget an appointment you booked weeks ago. Since the services are reserved for your Pet personally, we kindly ask for 24 hours notice of a cancellation.

 

Cancellation Day of Appointment:  50% of the cost of service(s).
No call/No show:  Full cost of the service(s).
